TO PRINT THE ENTRY FORM:

Windows users:

1) RIGHT click your mouse anywhere in the field blow.

2) A drop-down menu will appear; select "PRINT PICTURE" by LEFT clicking on the phrase.

Mac users: Just drag the image to the desktop and print the usual way.
